Step 3
Organisational Pulse
Score each statement from 1 to 5. The Pulse identifies leading indicators of organisational risk, execution friction, leadership credibility and change adoption.
Respondent lens: answer from what you observe in your role and function. If you are unsure, use the closest evidence-based judgement.

Respondent details
Pulse commercial context
These inputs keep the Pulse report commercially grounded and allow Versa to separate submitted context from benchmark assumptions.
Required before submission
Fields marked with * are mandatory and stay part of the record even after the survey answers are completed. Missing details will be highlighted here before the report can be generated.
Data note: early pilots store respondent details, answers and scores for report generation, admin follow-up and quality control. If the Pulse is used with a wider employee population, anonymity should be agreed and configured before launch.
20 required details still missing.
3-minute taster assessment
Three scored questions test whether there may be hidden risks, blind spots or opportunities outside the current line of sight. If you select Agree or Strongly agree, add up to three examples so the signal has context.
TQ1
Issues slowing performance
I believe there are significant issues currently slowing the organisation down, reducing performance, or putting the business at risk.
What this means
This tests whether the respondent can see material drag on performance, customers, employees, delivery, cash, margin, risk or execution confidence.
TQ2
Underestimated risks
I believe there are significant business risks or operational challenges that leadership may be underestimating, not seeing, or not addressing sufficiently.
What this means
This surfaces blind spots or weak signals, including decision avoidance, cultural resistance, customer risk, operating constraints, governance gaps or capability issues.
TQ3
Performance priorities
I can identify important changes or actions where the organisation could improve overall performance, effectiveness, or its ability to execute successfully.
What this means
This identifies whether there are practical improvement hypotheses that could become follow-up workstreams, such as cadence, decision rights, customer focus, operating model or accountability.
Mini Pulse Report
Initial organisational signal
Submit the three scored taster questions first to produce an initial view. The full 62-question diagnostic is then unlocked by PIN for a deeper, scored board report.
Access pathway
Move from taster signal to full diagnostic evidence
The Mini Pulse is deliberately light-touch. The full Pulse is the paid diagnostic layer: it validates the signal across 62 weighted questions, produces the 4I pathway, creates the board report needed for an executive review, and turns the result into a practical advisory route.
Commercial route: Mini Pulse is the free signal, Full Pulse is the evidence layer, and Executive Review is where the findings are converted into action, ownership and value-protection validation.
Full Pulse access is released after payment, approval or pilot authorisation. The buyer/sponsor controls launch setup and report release.
Internal pilot bypass
A. Mini Pulse
Free taster
Three scored taster questions plus examples where risk is signalled create an early snapshot.
Useful for curiosity, validation and deciding whether a deeper diagnostic is worthwhile.
B. Full Pulse
Paid diagnostic
The 62-question weighted diagnostic produces tier scoring, 4I pathway, risk heatmaps, value-leakage evidence and a board report.
Useful when leadership wants evidence, not just opinion.
C. Executive Review
Bespoke advisory
Versa interprets the results, challenges the evidence and converts the findings into an intervention plan.
Useful when the organisation needs help moving from insight to measurable action.
